To make your first visit as smooth and efficient as possible, we have provided the necessary forms and information below. Completing these forms ahead of time helps us better understand your health concerns and allows us to focus more time on your care during your appointment.

Appointment Cancellation Policy
We understand that unexpected situations may arise and you may need to cancel or reschedule your appointment. If this occurs, we respectfully ask that appointments be canceled at least 24 hours in advance.
When a patient does not attend a scheduled appointment, another patient loses the opportunity to be seen.
If we do not receive notice within 24 hours, a $10 fee will be assessed for the first missed appointment. Additional late cancellations or missed appointments may result in a $35 fee.
The cancellation fee will be waived if the appointment is canceled and rescheduled for the same day.
We appreciate your understanding and cooperation as this policy helps us better serve all patients.

During your first visit, Dr. Erdmann will perform a comprehensive evaluation to better understand your health concerns and develop a personalized care plan.
Your visit may include:

- Physical examination
- Functional assessments
- Recommendations for treatment
